A part of our service is to file the original certificate of death with the Oregon Health Division at the county health department in which the death has occurred.  The information that we need for that purpose from either the family or from your pre-arranged funeral plan file is listed on the following form.

A critical component in the process of arranging for cremation is written authorization from the person (or persons) with the right to control the final disposition of a body.  At the bottom of the cremation authorization form there is a list of different classifications of people who may have the right to authorize cremation.  That list is presented in the order of who has priority in giving this authorization, starting at the top with those with the most priority.  It is possible that there are several poeple with an equal right.  In that event it may be necessary to have more than one form prepared so that each person with an equal right has an opportunity to give their written authorization.  Please pay particular attention to the portion of this form where the funeral home is being instructed on what to do when releasing the cremated remains (ashes) from our possession.  We must carry out our instructions exactly with no exceptions or variations in these instructions.

The following forms follow the standard format of the Daily Astorian newspaper obituary format.  Death Notices and Obituaries are published at no charge by this newspaper with the understanding that there is no deviation from this format.  There are many other policies to follow with regard to obituary content that are too numerous to mention here.  Typically speaking, what the families expect is also what the Astorian publishes, but when this is not the case the family can purchase a paid obituary and have it appear as they want.  In a paid obituary the content is entirely up to the family and they can even submit more than one photo and select the day of publication.  The following form can be filled out or simply used as a guide for a basic obituary.  Photos are encouraged.  If the photo is a group photo, usually computer software can zoom in, crop, touch up, etc.  It can be difficult to use a photo that requires us to zoom in and remain in focus on a distant shot rather than a close up.
